Contested Diagnoses is a full-length clinical reference that takes on a question most practitioners learn to work around rather than confront: which parts of the DSM-5 rest on science, and which rest on convention? Spanning forty-one chapters, the book treats the diagnostic manual not as settled fact or corrupt artifact, but as what it actually is ? a large, consequential, imperfect document produced by a profession working at the edges of its own knowledge, shaped by institutional and cultural pressures that don't always align with scientific accuracy.

The book opens with five foundational chapters that anchor everything that follows: how the DSM-5 was built and what its architects intended, a working typology of the different kinds of diagnostic controversy, the validity problem underlying all psychiatric classification, the blurred boundary between ordinary suffering and disorder, and the financial relationships between the manual's authors and the pharmaceutical industry. From there, the book moves through disorder-specific controversies in depth ? from Major Depressive Disorder and its eliminated bereavement exclusion, to the pediatric bipolar epidemic and its DMDD correction, to the unresolved debate over how gender dysphoria should be classified, to the paraphilic disorders and their forensic uses. A final section addresses cross-cutting structural issues: the tendency of DSM criteria to miss female presentations of common conditions, the Western cultural assumptions embedded in categories applied worldwide, the outsized consequences clinical diagnoses carry in forensic settings, the patient perspective the diagnostic enterprise often overlooks, and the competing frameworks ? RDoC, HiTOP, ICD-11 ? actively vying to reshape psychiatric nosology. The book closes by considering what a DSM-6 that genuinely learned from these debates might look like.

Each chapter follows a consistent structure: a clinical vignette that grounds the controversy in a realistic patient encounter, historical and scientific background, the strongest arguments on both sides of the dispute, a summary of the relevant empirical evidence, and a section on clinical implications. Throughout, the book distinguishes empirical disputes ? ones more and better research might eventually resolve ? from values disputes, where no amount of evidence will settle the disagreement because the disagreement was never really about evidence in the first place.

Written for clinicians in active practice, researchers and scholars who work with DSM categories as their primary unit of analysis, and graduate students and trainees who will inherit this diagnostic system, Contested Diagnoses is designed to serve each audience on its own terms: clinical implications sections for working practitioners, curated further-reading lists for researchers, and Key Takeaways summaries for students.
